Schedule: Day shift, rotating weekends (currently every 4th weekend)

 

Position Overview:
We’re seeking an experienced Home Health Registered Nurse for a 13-week assignment in the Palm Desert, CA area. The ideal candidate is flexible, organized, and compassionate, with a strong background in delivering high-quality patient care in a home-based setting.

 

Responsibilities:

• Provide comprehensive nursing care to patients in their homes within a 20–25-mile radius.
• Conduct patient assessments, develop care plans, and evaluate outcomes.
• Coordinate with multidisciplinary teams to ensure continuity of care.
• Complete documentation using HCHB and OASIS systems (training available if needed).
• Rotate weekends as scheduled (every 4th weekend).

Requirements:

• Active California RN license
• BLS certification (AHA)
• Minimum 2 years of nursing experience
• 2 years of Home Health experience required
• Experience with OASIS and HCHB preferred (will train if needed)
• Reliable transportation (mileage reimbursed at $0.70/mile)

Additional Details:

• No on-call requirement
• All RTO must be approved in advance and submitted with application
• Extension opportunities may be available
• Scrubs: Any color
• EMR: HCHB / OASIS
• Parking: Provided at no cost
