Reedsburg Area Medical Center is recruiting a Social Worker to join our team! Position Summary: The Social Worker assists the patients at the Reedsburg Area Medical Center hospital and clinics. These tasks could include but are not limited to discharge planning arrangements, i.e. nursing referral, equipment, nursing home/assisted living, hospice, ADRC, meals/housekeeping; financial issues; complaints, concerns and advocacy. The Social Worker is aware of community resources and communicates resources depending on needs of patient. The Social Worker assist patients to deal with emotional issues, i.e. counseling services, finances, support groups. The Social Worker at RAMC works with all age population and is knowledgeable on emotional, spiritual and social aspects that impact their lives. Job Relationships: Reports to Director of Med Surg Unit and collaborates with Discharge Planning Team in the acute care setting. Primary Customer Served: Neonatal (Birth – 30 days)Infant (31 days – 1 year) Pediatric (1 – 12 years) Adolescent (13 – 18 years) Adult (19 – 65 years) Geriatric (> 66 years) Family/Visitors Physicians Community Agencies Social Work Team Education and Training Required: BSW or related field. If Bachelors in related field, must obtain Social Worker certificate. Licensure in the State of Wisconsin. Experience: One (1) year in hospital, nursing home or related health field assisting patients and residents preferred. Special Knowledge, Skills and Abilities: Relates well to people. Works independently and under time constraints. Organizes work and priorities. Documents in a timely manner. Knowledgeable of computer system. Knowledgeable of standards, regulations and codes, applicable to Social Service activities. Physical Requirements: Physical Demands:Able to sit, stand and walk during the course of a normal work day.On occasion may need to bend or squat to retrieve items or supplies, lift, push, pull or carry up to 10 pounds. Items greater than that may be delegated out to another individual.Able to spend extended periods of time on a telephone with other facilities. Occasionally work over allocated hours on a normal shift. Sight: Near and far vision is required in order to visualize computer screens, review documents, and assist patients and other members of the healthcare team. Cognition/Communication: Ability to participate in all aspects of communication in regards to writing, reading, math, hearing and comprehensive speech skills. Has minimal exposure to complex math skills. Environmental Conditions: Is continually exposed to an inside, patient care environment with occasional exposure to unpleasant odors. Potential Hazards:N/A Psychological, Situational, Functional Factors:Able to multitask, remain flexible .Maintain interpersonal skills, deep conversation, work effectively alone, take initiative and handle stressful situations.
