One of our client, a Health Center located in New York, is looking for an School Program Nurse Practitioner (Perm).
Position Type: Full-Time/Perm
Location: New York
Schedule: Weekdays Mon-Fri 08:00am-04:00pm
Job Summary - Under the direction of the Program Coordinator oversees integration of School Health services within the site.
- Provides comprehensive case managed primary care services in an ambulatory care setting at the school and Health Center.
- Contributes to quality assurance, staff development, and other programmatic and clinical functions.
- Provides telephone coverage, based on the Health Center's on-call schedule.
Job Duties - Administer care to ill, injured or disabled school aged children
- Advise patients on health maintenance and disease prevention and provide medical case management.
- Maintain accurate, detailed reports/records and participate and oversee performance and quality improvement activities.
- On-call responsibilities - telephone coverage 3 months total, sharing coverage for on call demand. Light call load maybe once a week at most, majority from the lab or ED.

- Familiar with school health of mainly underserved population and immigrants
- Responsible 700-1500 over the course of the year, 12 avg patient visits per day, all elementary school aged
- If less than 3600 hours worked as NP, would need strict more collaborative agreement
- Ability to work alone and not needing a collaborating physician when seeing patients
- Will at times float between school and Client's main center, 1-2 time a year
- Lots of opportunity to build/create programming
- Familiar and a desire to work in a community health setting
- Excellent written, verbal, and interpersonal skills, two-to-three-year experience in healthcare, supervisory and administrative roles
- Bilingual - Spanish - Preferred
Qualifications - Current NYS NP License
- Current PNP Board Certified
- CPR Certification
- Infection Control Certification
- NYS Provider Medicaid and Medicare # required
- COVID Vaccination Required
- DOE fingerprinting required
