We are seeking a compassionate and experienced Home Health Caregiver to provide dedicated care for a patient with dementia. The ideal candidate will have a strong background in dementia care and a passion for improving the quality of life for patients and their families. This role involves providing personal care, companionship, and assistance with daily activities to ensure the comfort and well-being of our patient.

Key Responsibilities:

• Provide personalized care and support to a dementia patient in their home
• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Administer medications as prescribed and monitor health changes
• Prepare nutritious meals and assist with feeding as needed
• Engage the patient in meaningful activities to stimulate cognitive function and enhance quality of life
• Maintain a safe and clean environment in the patient's home
• Offer emotional support and companionship to the patient and their family
• Document and report any changes in the patient's condition to healthcare professionals
• Collaborate with the healthcare team to develop and follow individualized care plans

Qualifications:

• Minimum of 5 years of experience in home health care, specifically with dementia patients
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
• Knowledge of dementia care techniques and best practices
• Excellent communication and interpersonal skills
• Patience, empathy, and a strong sense of compassion
• Ability to handle physically demanding tasks and maintain a high level of energy
• Reliable transportation and a valid driver's license
• Ability to pass a background check and drug screening
