*Job Summary:*

We are seeking a talented nurse practitioner or physician assistant to join our rapidly growing medical practice. As our full-time NP or PA you provide medical care at skilled nursing facilities; focused on geriatric and rehabilitation patients. The ideal candidate will have strong clinical, interpersonal skills and a team based mentality. The candidate will be able to work independently and as part of a team to provide excellent patient focused medical care. New Graduates welcome and/or willing to train the right individual.

*Responsibilities:*
* Acute patient care visits, medical recertifications
* coordinate and provide excellent and appropriate patient care
* Collaborate with administration, nursing, therapy and families
* this is a post-acute care setting

*Qualifications:*
* NP or PA degree from an accredited school
