*Overview*

We are currently recruiting an experienced Nurse Practitioner with an active license in the _*central & northern Illinois region*_. This is a Part-Time/Full-Time position in a SNF facility/facilities.

*In this role, your responsibilities will include:*
* Patient visits
* Follow the approved care plan for each patient
* Ensure exceptional inpatient care in a post-acute care setting
* Patient volume is approximately 20-25 patients each day

*Qualifications*
* Nurse Practitioner degree from an accredited nursing school
* Active state license and DEA license in good standing
* 1 year of experience as a Nurse Practitioner (post-acute care experience preferred)
* Proficiency with EMR
* Compassionate, Intelligent, Motivated, Team Player

Job Types: Full-time, Part-time

Pay: $111,000.00 - $125,000.00 per year

Standard shift:
* Day shift

Weekly schedule:
* Monday to Friday
* Weekends as needed
