Compensation: $41.50 - $50.00 per visit

Position Summary: Home Health Care, Inc. is seeking a Social Worker - MSW to join our team and provide care for our clients throughout the Twin Cities metro area! Your role as a social worker is crucial to our clients because the importance of your knowledge of resources and aspiration to help clients will create long-term gratification. This role offers much fulfillment in the way of making a direct impact on our client’s personal achievement and goals to live a healthy lifestyle.

Responsibilities:

• Make Social Work home visits to assess patients' psychosocial status
  
• Provide Social Work services in the home, including short-term individual counseling, community resource planning, and crisis intervention
  
• Work closely with all members of the health care team in the development of the plan of treatment
  

Qualifications:

• Licensed in the state of Minnesota as an LGSW or LISW (Master's-Level Social Worker)
  
• At least one year of clinical Social Work experience
  
• Experience with Electronic Medical Records (EMR) preferred
