Reporting to the Chief Care Services Officer, ALS United Greater New York is seeking an experienced licensed social worker to provide comprehensive support services and coordination of multi-disciplinary care services to help improve the quality of life of patients and their families living with ALS on Long Island in Nassau County and the borough of Queens. ALS is a fatal and neurodegenerative disease that requires a compassionate and committed social worker who is comfortable with end of life care.

In collaboration with other members of the LI and NYC care teams, this full-time, home-based position will conduct virtual and in-person home visits offering social work assessments, short-term supportive counseling, case management, and crisis intervention. This position will also require the co-facilitation of support groups for patients and caregivers, as well as coordination of monthly educational programs all of which, at present, are being conducted virtually. In addition, the position requires periodic participation in chapter-wide events, some of which take place on weekends and evenings.

QUALIFICATIONS:
LSW with active NY State license
At least three years experience in similar role
Experience in end of life care
Strong communication and interpersonal skills
Strong team player
Bi-lingual a plus
High degree of initiative, able to work independently
Experience in group work
Valid drivers license and use of car
Job Type: Full-time
Pay: $75,000.00 - $85,000.00 per year
