Job Description Job Description Description: Reports to: Coordinator of Individualized Services Rate of pay: $21.59/hour; paid biweekly; sign-on bonus available Work schedule: Full-time, 37.5 hours per week. Primarily Monday - Friday, day-shift; On-call requirement; Schedule variability as required by needs of the individuals supported Position Summary: Assist Coordinator in carrying out and assuring compliance of daily habilitation operations and all services as set forth in service plans, agency policies, regulations, Quality Assurance, and individual choice and provide direct support to individuals. (Approximately 30% of job responsibilities involve direct support.) This position is located at our Norwood, NY IRA house. Primary Responsibilities: Provide direct support and habilitation services for each individual. Assist with and/or provide emergency and fire safety training, maintain the rights of each individual, and assist with making informed choices. Assist individuals with personal care skills to promote self-sufficiency through increasing independence, productivity, and inclusion in the community. Assist individuals with, but not limited to, decisions relating to nutrition, budgeting, and community activities. Assist individuals by promoting independence in medical appointments, medication administration, and transportation through education to ensure best possible health. Assist weekly with maintaining wages, personal and household funds, and assuring receipt and accountability. Assist with communication with educational/vocational/day programs, DDSO, families, and other appropriate persons in line with programming goals. Supervise and coordinate the daily activities of direct support professionals, including ensuring adequate coverage, managing time off requests and schedules, and effectively evaluating staff. Prepare and update staff action plans at least every six months or earlier as needed. Monitor billing sheets and notes weekly, ensuring they are complete and accurate. Report immediately all potential abuse and incidents per OPWDD regulations. Complete monthly summaries per agency policy and remit to the Coordinator. Complete Investigator training as requested by the Director of Residential Services and conduct investigations as assigned. Complete Personal Outcome Measures training and conduct interviews as assigned. Work a flexible schedule, including weekend, evening, and overnight coverage when necessary. Participate in 24-hour on-call system. Perform other duties as requested by the supervisor. Requirements: High School diploma or GED required. One year of experience in MR/DD field is preferred. One year of supervisory experience is preferred. Ability to pass all OPWDD-required background checks. Successful completion of Medication Certification training program. Reliable personal vehicle for business use. Valid NYS Driver’s License (2 years’ driving experience required). Acceptable motor vehicle abstract to meet Agency insurability standards. Ability to lift and carry up to 25 lbs, lift and support individuals with and without help, and shovel snow. Physical strength to safely respond should a behavioral crisis occur. Ability to operate personal and Agency vehicles. Ability to walk and stand for extended periods. Some exposure to infectious diseases, hygiene issues, food, and other odors. Able to tolerate atmospheric conditions when performing regular outdoor seasonal activities. Ability to operate mechanical devices used by individuals. Sign-on bonus details: $1,000 total divided into four equal installments that are paid out at 90 days, 180 days, 170 days, and one year of employment Benefits: Cerebral Palsy Association of the North Country offers eligible employees excellent, affordable benefits options.
