Interim HealthCare of West Michigan  is seeking an independent and detail-oriented nurse to join our Intake team in the Grand Rapids office. Our Intake team works with hospitals, facilities and community physicians to process referrals for patients signing on to Hospice. Intake RN/LPN Responsibilities  

• Acts as a liaison between referral sources, external agencies, health professionals and field staff 
• Receives and coordinates hospice admissions, screening calls and referrals to assist in accessing appropriate services 
• Communicates with the patient and family regarding arrangements for initiation of services 

Pay rate  

• LPN: starting at $50,000 
• RN: starting at $60,000

Minimum Requirements  

• Registered Nurse or Licensed Practical Nurse with active, unrestricted license in the State of Michigan – BSN not required for RN candidates  
• At least one (1) year of clinical experience as a Registered Nurse or LPN 
• Excellent written and verbal communication 
• Ability to multitask throughout the day 

Preferred Skills  

• One (1) year of Home Health or Hospice experience
