Job Description
POSITION SUMMARY: Provides comprehensive nursing assessment of patient with Diabetes including planning, implementation, coordinating, monitoring and evaluation of a plan of care. Serves as a resource for Diabetes care across the continuum of care. Functions as a practitioner, consultant, role model, and educator, ensuring quality patient care and outcomes. Coordinates optimal resources within the health care team of physicians, nurse managers, clinical specialists, patients and families. Participates in continuous quality improvement activities and educational experience in support of departmental and divisional nursing philosophy and objectives, as well as, Health System initiatives.
EDUCATION/TRAINING: Bachelor of Science in Nursing and current Registered Nurse licensure from the Illinois Department of Professional Registration. Certification as a diabetes educator with experience in education preferred.
LICENSURE/CERTIFICATION: Current Registered Nurse licensure from the Illinois Department of Professional Registration. Annual BLS certification required.
EXPERIENCE/SKILLS/ABILITIES: Excellent interpersonal skills and ability to effectively deal with all levels within the organization and community. Strong verbal and written communication, organizational and critical decision making skills. Successful completion of annual clinical, age specific and job specific competencies and skills validation tools.
