Responsibilities - Develop diabetes self-management education and pre-diabetes related education resources for patients referred for consultations. This includes patient assessment and education on diabetes self-management
- Provides education and training for care managers, medical assistants and nursing staff regarding diabetes self-management using the 3 M of self-care and other care plans/resources in alignment with ADA guidelines.
- Assess clients' nutritional, health needs and social determinant needs
- Conduct in person, Televideo and groups education and counseling
- Participates in community events on the topic of healthy eating and diabetes education.
- Documentation care plans in Epic
- Evaluate if the meal plans have a positive effect on clients' health
- Create educational material about healthy eating habits
- Speak to groups about the importance of nutrition to prevent specific diseases
- Document patient progress
Qualifications Job Requirements:
- Certification as a Diabetes Educator by the American Diabetes Association
- Registered dietitian in the state of OklahomaOR
- Registered nurse in the state of Oklahoma
Preferred Job Requirements:
- 3 years of experience as a dietitian or RN
