Job Purpose
The purpose of the Community Health Worker of the Emergency Food Bank (EFB) organization is to execute the education and food distribution programs for our Future Well Communities collaborative effort, our Food-As-Medicine Programs, our Nutrition Education Programs, our Community Outreach Programs, our Street Outreach to the Homeless Encampments Program and the On-Site Nutrition Education Center. The Community Health Worker, working with the Nutrition Educators, shall travel to sites to execute outreach, gather and register participants, deliver nutrition education and teaching, and participate as needed in all the activities of the Programs . This work involves developing, teaching, and coaching the monthly recipe and delivering a short, 15-minute cooking demonstration or nutrition education class with emphasis on friendly, courteous service to clients and volunteers. On average 2 of the 65 sites are visited daily. Additionally, Nutrition Education activities occur daily at the EFB. There are weekend Health Fairs and other Community Activities that the CHW would attend as part of the Nutrition Education Outreach conducted by the EFB. This position requires driving the Mobile Farmers Market van. This is a full-time (35 hours/week), non-exempt position. The position is funded by a San Joaquin County Contract, in concert with the SJC Public Health Department, that is in effect from 1 September 2022 to 30 August 2025. The position could end at that time or the County may issue a new contract.
Duties and Responsibilities
Typical essential duties may include, but are not limited to:
- General Responsibilities
Operate the MFM van/program and the educational classes. Driving the van and making site stops.
Organize the MFM merchandise, pallets, produce, cleanliness/cleanup and volunteers working that day.
Assist the Nutrition Educators in developing curriculum, recipes, and materials.
Learn and use the UniteUs Platform
Learn and use the Community Connected Network (CCN)
Learn and use the HMIS system.
Work collaboratively with other CBOs in Outreach, Street Outreach, and Nutrition Education
Represent EFB by attending the CCN and UniteUs meetings, collaborate with all CCN members and be active with UniteUs.
Collect and maintain grant/agency required reporting information.
Work in concert with the Office Manager on grant/agency reporting.
Lead in the accurate gathering and maintaining of intake records for the EFB Main Pantry.
Work in cooperation with MFM Assistants and Nutrition Educator, supply necessary training and daily guidance.
- Operational Responsibilities
Present the MFM program to clients at various off-site locations throughout San Joaquin County.
Organize and arrange all equipment (tables, produce boxes) and supplies (bagged produce, brochures) required for set up at various locations.
Set up, prepare, and deliver cooking demonstrations and/or nutrition education per site.
Weigh and inventory all produce leaving and returning food to the Emergency Food Bank per site.
Properly clean all equipment and supplies, and return course materials/course food to the EFB.
Teach general nutrition education classes to on-site and off-site clients.
Maintain, set up and collect client records and surveys for grant purposes.
Help/fill in with other EFB work requirements when work load requires, as directed by the Warehouse Manager, the Office Manager, or the CEO.
Attend Community/Organization Health Fairs as a representative of the Emergency Food Bank. Talk about and distribute information about the Organizations services and programs.
Additional hours may be required occasionally during the Organizations activities and events, including holidays, evenings, and weekends.
Perform other duties as assigned.
Qualifications

Have a good working knowledge of Microsoft Office (Excel and Word).
Be able to learn and use database Programs such as Oasis, as well as the HMIS system.
Be able to learn and use UniteUs Platform and CCN.
Able to communicate clearly and effectively with all EFB staff, volunteers, clients, and general public.
Bilingual Spanish is required; it is desirable if the CHW can translate during presentations
Valid Class C California Drivers License and proof of insurance, and provide DMV printout.
Education and Experience
High school diploma and some training/coaching experience, or combination of education and desire to learn how to become a successful nutrition educator
Fork lift and warehouse experience a plus, but not required.

Continual repetitive lifting, lowering, carrying, and/or moving up to 50 pound job related equipment and materials to a height of 3-4 feet and load them into EFB vehicles, shelves, or bins.
Specific vision abilities required by this job include close vision, distance vision and depth perception.
The position constantly works in outdoor weather conditions.
