Our leadership has over 25 years of experience in the skilled nursing space, and we've applied our extensive industry knowledge to our proprietary EMR system, in-facility treatment modalities, and insurance-compliant documentation.
Role:
The Clinical Liaison is responsible for maintaining and ensuring the overall delivery of services and maintaining clinical care standards for affiliated facilities. The liaison collaborates closely with wound care providers and the management of affiliated facilities to provide consistent quality care for patients and wounds, enhance customer loyalty, and exceed client satisfaction.
Duties and Responsibilities:
- Serve as liaison between ReNew and each of the assigned centers
- Visit assigned centers every 2-4 weeks, depending on the need of the center
- Periodically attend wound rounds with the provider and wound team to ensure the process is correct
- Act as a clinical resource for the center, providing ongoing wound care education
- Educate new providers on the wound rounds process and facilitate onboarding of new providers in assigned centers
- Review the clinical outcomes / wound analysis of assigned facilities using Casper Reports and QAPI reports.
- Provide mock surveys for assigned centers to help prepare for state inspections
- Collaborate closely with facility wound nurse and nursing staff on wound management practices
- Attend quarterly QAPI meetings for assigned facilities
- Work with the wound care nurse and facility leadership to determine opportunities for education – such as appropriate identification of wound types, prevention of wounds, and nursing care of wounds.
- Develop wound care education plans with the facility wound care nurse and facility leadership.
- Participate in professional development activities and maintain professional affiliations.
- Complete the required number of continuing nursing education hours for license and certification.
- Attend required meetings or committees as requested
- Maintain patient confidentiality
- Perform other duties as determined collaboratively with the team and administrators within the scope of practice.
- Maintain a copy of all professional records, including current licensure, certification, continuing education, and other appropriate records to practice.

Requirements :
- Bachelor's Degree in Nursing or Graduate of an accredited Licensed Practical Nurse Program.
- Minimum of two to three years experience in the clinical field; SNF experience preferred
- Wound certified preferred
Physical/Mental Demands and Work Environment:
- Involves exposure to communicable diseases, medical prescriptions, toxic substances, and other conditions
- Requires full range of body motion to get in and out of the car, as travel is required
- Frequent standing, walking, and sitting required for extended periods of time
- Requires visual and hearing acuity
- Able to lift a minimum of 25 pounds
