Adult/Geriatric of Family Nurse Practitioner needed for in-house coverage in Subacute/Skilled/Long Term Care facility in Cherry Hill, New Jersey. Will be responsible for attending daily morning meetings, rounding on acute issues and following up on lab studies, x-rays, recommendations, and more. Assisting nursing staff in recognition and early intervention in acute illnesses with the goal of reducing unplanned transfers to the hospital. NP will participate in facility programs such as antibiotic stewardship, QAPI, and will be actively involved in nursing education. This position is in collaboration with a physician and other NP’s who are not on site. We will not be the primary care providers of these patients and will coordinate all care with the primary doctors.

Ideal candidate will have experience with long term care or older population and managing multiple chronic diseases. Interest in or experience with Psychiatric patients is a plus.
